LHSC provides care in the community
Most Londoners visit our hospitals at some point in their life, but did you know that London Health Sciences Centre (LHSC) also provides care in community-based settings?
Victoria Family Medical Centre and Byron Family Medical Centre
LHSC operates two family medical centres that provide primary health care to registered patients through a comprehensive, interdisciplinary approach. Each centre’s team consists of family physicians, nurses, physiotherapists, social workers, and psychologists. Each centre offers select ambulatory care services, including minor surgical procedures.
Kidney Care Centre
Located at the Westmount Shopping Centre, LHSC’s Kidney Care Centre offers comprehensive care to patients with chronic kidney disease. The Kidney Care Centre includes:
- Chronic kidney disease clinics and education, including a pre-dialysis clinic focused on slowing the progression of chronic kidney disease;
- Outpatient hemodialysis treatment;
- Peritoneal and home hemodialysis assessment, training, and maintenance; and
- An Indigenous resource room.
54 Riverview Ave.
LHSC’s Adult Eating Disorders Service, provided in partnership with the Canadian Mental Health Association Middlesex (CMHA Middlesex), is located at 54 Riverview Ave. The facility allows patients to receive treatment under one roof and includes a residential treatment program for those requiring intensive support. 54 Riverview Ave. will soon also become home to a new location for LHSC’s First Episode Mood and Anxiety Program – a novel outpatient mental health program that provides treatment to emerging adults with mood and anxiety symptoms. 54 Riverview Ave. includes eight Youth Transitional Apartments supported by CMHA Middlesex. These independent-living, residential apartments are for young adults who are transitioning back to the community.
